Where to find the payment address
Check the statement first, especially near the payment coupon. Many offices also list a payment link on their website or on the patient portal. If you only have a phone number, call and ask which website to use.
What you will usually enter
- The account or statement number
- The patient’s name, and sometimes date of birth
- A zip code or other identifying detail
- A card or bank payment method
Check before you pay
